What This Medicine Does
This combination antibiotic works by blocking two key steps in how bacteria make folic acid—a nutrient they need to grow and multiply.
- Sulfamethoxazole stops bacteria from making dihydrofolic acid
- Trimethoprim blocks the next step: converting dihydrofolic acid into tetrahydrofolic acid
- Together, they prevent bacteria from making DNA, RNA, and proteins, which kills or stops their growth
What It Treats
This medicine is effective against a wide range of bacteria and some protozoa, including:
Commonly Treated Infections
- Urinary tract infections
- Respiratory infections
- Gastrointestinal infections
- Skin and soft tissue infections
- Pneumocystis pneumonia (especially in people with HIV/AIDS)
Active Against These Germs
- E. coli, Salmonella, Shigella, Klebsiella, Enterobacter
- Streptococcus, Staphylococcus, Pneumococcus
- Haemophilus influenzae, Neisseria, Vibrio cholerae
- Bordetella pertussis, Proteus

Important Safety Precautions While Using Sulphamethoxazole and Trimethoprime Injection
Preventing Drug Resistance
- Only use this medicine for confirmed or strongly suspected bacterial infections.
- Using it unnecessarily can lead to drug-resistant bacteria, making future infections harder to treat.
Folate Deficiency Risk
- Use with caution in people who may have low folic acid levels, such as:
- Elderly adults
- People with poor nutrition or alcohol use
- Those on seizure medications or with digestive issues
- Blood changes may occur but can be reversed with folinic acid.
Blood and Infusion Reactions
- People with G6PD deficiency may experience blood cell breakdown (hemolysis).
- If the IV drip causes redness or swelling, stop and restart at a different site.
Low Blood Sugar
- Rare cases of low blood sugar have occurred, especially in people with:
- Kidney or liver problems
- Poor nutrition
- High doses of the medicine
Other Conditions
- Use with caution in people with thyroid issues, porphyria, or phenylketonuria (PKU).
- Trimethoprim may affect phenylalanine metabolism, but it’s not a concern for those on a proper PKU diet.
Special Considerations for AIDS Patients
- People with AIDS may have more side effects like rash, fever, low white blood cells, and liver issues.
- High potassium levels (hyperkalemia) are more common—monitor closely.
- If a rash or reaction occurs, recheck treatment.
- Avoid combining this medicine with leucovorin during treatment for Pneumocystis pneumonia.
Fluid Intake & Kidney Health
- Drink plenty of fluids to prevent crystals in the urine or kidney stones.
- People who process medicine slowly (“slow acetylators”) may be more sensitive to side effects.
Patient Advice
- This medicine treats bacterial infections only—not colds or viruses.
- Diarrhea is common and usually goes away after stopping the medicine.
- If you get bloody diarrhea or stomach pain, even weeks later, see a doctor immediately.
Dosages administration of Sulphamethoxazole and Trimethoprime Injection
Sulphamethoxazole and Trimethoprime Injection Dosage for Children and Adults
Pneumocystis Jiroveci Pneumonia (PJP)
- Daily dose: 15 to 20 mg per kg of body weight
- Split into 3 or 4 doses given every 6 to 8 hours
- Treatment lasts up to 14 days
- In some adults with healthy kidneys, 10 to 15 mg/kg may be enough
Severe Urinary Tract Infections & Shigellosis
- Daily dose: 8 to 10 mg per kg of body weight
- Split into 2 to 4 doses given every 6, 8, or 12 hours
- Treatment lasts:
- Up to 14 days for urinary infections
- 5 days for shigellosis
- Maximum daily dose: 60 mL
For Patients with Kidney Problems
- If kidney function is reduced, the dose must be adjusted.
- Doctors use a special table to decide the right dose based on creatinine clearance (a measure of kidney health).

- People with AIDS may have more side effects when treated for Pneumocystis pneumonia.

- Infusion reactions (redness or pain at the IV site) are rare but possible.
- Cross-sensitivity may happen with similar drugs like diuretics or diabetes medications.
