General Safety
- Not a beta-blocker: If you’re stopping a beta-blocker, do it slowly over 8–10 days. Nifedipine won’t protect you from sudden withdrawal effects.
- Can be combined with other blood pressure medicines, but this may cause dizziness or fainting when standing up (postural hypotension).
Heart Health
- Use with caution if you have:
- Weak heart function (poor cardiac reserve)
- Heart failure,it may get worse in some cases
- Very low blood pressure (systolic below 90 mmHg)
- Severe narrowing of the heart valve (aortic stenosis)
- Some people may feel chest pain within a few hours of starting the medicine. If this happens, stop taking it and talk to your doctor.
Blood Pressure Monitoring
- Dialysis patients with high blood pressure and low fluid levels may experience a sharp drop in blood pressure.
- Blood pressure should be closely watched if you’re also taking IV magnesium sulfate, especially during pregnancy.
Pregnancy & Breastfeeding
- Pregnancy: Only use nifedipine if absolutely necessary, such as in severe high blood pressure that doesn’t respond to other treatments.
- Breastfeeding: Not recommended, as the drug can pass into breast milk and its effects on babies are unknown.
Diabetes
- If you have diabetes, your blood sugar control may need adjusting while on nifedipine.
Liver Function
- If you have liver problems, your doctor may need to lower your dose and monitor you more closely.
- Nifedipine hasn’t been fully studied in people with severe liver disease, so extra caution is needed.
Drug Interactions
Nifedipine is broken down by an enzyme called CYP3A4. Some drugs can affect how nifedipine works by increasing its levels in your body. These include:

For Heart Conditions
Chronic Stable Angina (Chest Pain from Effort)
- Start with: 10 mg twice a day (morning & evening) Or 20 mg twice a day
- Max dose: Up to 60 mg per day, increased slowly if needed
- If there is no improvement after 14 days, your doctor may switch you to a different form (capsules)
For High Blood Pressure (Hypertension)
- Start with: 10 mg twice a day Or 20 mg twice a day
- Max dose: Up to 60 mg per day, adjusted gradually
- If you’re taking certain medicines (CYP3A4 inhibitors or inducers), your dose may need to change—or nifedipine may not be suitable
How to Take It
- Swallow tablets whole with a little water
- Do not chew or crush
- Take every 12 hours (e.g., 8 AM and 8 PM)
- Never take doses less than 4 hours apart
- Avoid grapefruit juice—it can affect how the medicine works
- Can be taken with or without food
