Depends On:

  • Severity of the infection
  • Type of bacteria causing the infection
  • Patient’s age, weight, and kidney function

Adults (with normal kidney function)

  • Usual dose: 1–2 grams every 12 hours
  • Route: Intravenous (IV) or Intramuscular (IM)

Elderly Patients

  • No dose change needed unless kidney function is reduced

Children

  • Dose: 50 mg per kg of body weight.
  • Frequency: Every 8 to 12 hours.
  • Note: Not recommended for children under 12 due to limited data.

Patients with Kidney Problems

  • Cefpirome is cleared through the kidneys, so

How to Prepare and Give Cefpirome with Sulphate Injection 1000 mg

Intravenous Injection (IV Push)

  • 1g vial dissolved in 10 ml sterile water.
  • Inject slowly over 3–5 minutes.

Short IV Infusion

  • 1g vial dissolved in 100 ml of
    • Sterile water.
    • 0.9% sodium chloride (saline)
    • Ringer’s solution.
    • 5% or 10% glucose.
    • 5% fructose.
    • 6% glucose + 0.9% saline.
  • Infuse over 20–30 minutes.
  • Do not use sodium bicarbonate solution

Handling Tips

  • Tilt the vial gently for 1 minute to dissolve (foaming is normal)
  • Use aseptic technique to avoid infection.
  • Use the solution immediately after preparation.
  • If stored:
    • Up to 6 hours at room temperature.
    • Up to 24 hours in the fridge (2°C to 8°C)
  • Protect from sunlight.

Hematologic Changes.

  • Thrombocytopenia.
  • Eosinophilia.
  • Hemolytic anemia (very rare)
  • Granulocytopenia and agranulocytosis (especially with prolonged beta-lactam use)

These are often Type II or Type III hypersensitivity reactions, involving antibody-mediated cell destruction or immune complex deposition.
