Cefazolin is a cephalosporin antibiotic used via injection to treat various bacterial infections. It works by blocking bacterial cell wall synthesis, effectively killing the bacteria (bactericidal action).
How Cefazolin Injection 1000 mg Works ?
When cefazolin is injected—either intramuscularly (into a muscle) or intravenously (into a vein)—it enters the bloodstream and begins its journey to fight bacterial infection.
Cefazolin Injection USP 1000 mg is given intramuscularly or intravenously at the same total daily doses for both routes.
Adults:
Mild infections: 250–500 mg every 8 hours.
Acute, uncomplicated UTIs: 1 g every 12 hours (intramuscular only for this frequency).
Moderate to severe infections: 500 mg–1 g every 6–8 hours.
Severe infections (e.g., endocarditis): Up to 6 g/day in divided doses.
Beta-hemolytic streptococcal infections: Continue for at least 10 days.
Renal impairment:
Start with a loading dose, then adjust:
Creatinine clearance ≤ 0.91 mL/s (≥ 140 µmol/L): 250 mg–1 g every 6–12 hours.
Creatinine clearance 0.58–0.90 mL/s (141–273 µmol/L): 250 mg–1 g every 8–12 hours.
Creatinine clearance 0.18–0.57 mL/s (274–406 µmol/L): 125–500 mg every 12 hours.
Creatinine clearance ≤ 0.17 mL/s (≥407 µmol/L): 125–500 mg every 18 hours.
Surgical prophylaxis:
Pre-op: 1 g IV/IM 30–60 minutes before surgery.
During long surgery: 0.5–1 g IV/IM as needed.
Post-op: 0.5–1 g IV/IM every 6–8 hours for 24 hours (may extend to 3–5 days for high-risk procedures)
Children:
Mild to moderate infections: 25–50 mg/kg/day in 3–4 divided doses.
Severe infections: Up to 100 mg/kg/day.
Not for infants <1 month or premature infants
Renal impairment: Adjust dose as per guidelines.
Side effects of Cefazolin Injection USP 1000 mg have been reported, including the following:
Gastrointestinal:
Diarrhea, oral thrush, vomiting, nausea, stomach cramps, and loss of appetite may occur. Rarely, symptoms of pseudomembranous colitis can develop.
Allergic:
Infrequent allergic reactions, including anaphylaxis, itching, drug fever, skin rash, and eosinophilia (high eosinophil count).
Hematologic:
Possible effects include neutropenia, anemia, leukopenia, thrombocythemia, and positive Coombs’ tests.
Hepatic and Renal:
Temporary increases in liver enzymes (AST, ALT), BUN, and alkaline phosphatase may be seen, even without signs of organ impairment. Rarely, hepatitis or cholestatic jaundice can occur.
Local Reactions:
Occasional pain or induration at injection sites; phlebitis (vein inflammation) is rare.
Other:
Uncommon reactions include vulvar/genital itching, vaginal infections, and anal itching.
Cefazolin for Injection should be used carefully in people who are allergic to penicillin.
Some patients have had allergic reactions to both penicillins and cephalosporins, and in rare cases, these reactions can be serious or even life-threatening. Because of this, there is a risk of cross-reactivity between these two types of antibiotics.
If a patient has a known allergy, especially to drugs, cefazolin should only be given when really needed and with extra caution.
If a serious allergic reaction (anaphylaxis) happens, emergency treatment with epinephrine is necessary. Other treatments, such as oxygen, steroids given through a vein, and help with breathing (including intubation), may also be needed.
It is also possible for patients taking cephalosporins like cefazolin to develop a condition called pseudomembranous colitis.
This causes severe diarrhea and should be considered if diarrhea happens while using antibiotics.

Cefazolin for Injection comes in a clear glass vial that holds 10 mL of medicine. The vial is made of special USP Type III glass for safety and quality. It has a bromobutyl rubber stopper to keep the medicine clean and sealed. The top is covered with a flip-off aluminum seal to protect it until you’re ready to use it.
